I am the school nurse for Central High School as well as East Middle School (Tuesday and Thursdays), Cherry Knoll Elementary School, and Eastern Elementary School. I also provide on call coverage for the school district. If you call my office and I’m not available, please leave a message letting me know the best time to reach you and I will get back to you as soon as I can.
Children and health are important to me. My goal is to promote the health and well being of children to help them get the most out of their education. If your child has a chronic health condition, please make sure the school is aware of it. Update us during the year as needed.
Medication/Treatment Authorization Forms (get your form here)
Does your child use emergency medications for bee sting or food allergies, do they use an inhaler for asthma, take medicine for headaches? Complete this form, have the child’s primary care provider sign and return it to the secretary, along with the medication. It will allow your child to have medication at school to use it when needed. This is needed for prescription and over the counter medications.
Immunizations: Immunization Requirements
